Create Mechanical Turk or Qualtrics Survey

I am wanting to run a Mechanical Turk or Qualtrics study. I am looking for somebody to create the survey/game that I want to run either using the qualrics software or the survey capabilities that come on mechanical turk. Here is how the survey/game would work: 1. A qualtrics or mechanical turker agrees to do a hit. 2. A survey asks the worker for some basic information (age, race, gender, etc.) 3. The worker is told that they will play a simple button-pushing game. For 5 minutes, the worker must alternate between pushing the "a" key on the keyboard and then the "b" key on the keyboard as many times as possible. A clock must be present that is counting down from five minutes. A tally must also be present which shows the worker how many times they have clicked the "a" button and then the "b" button. 4. At the end of the study, I need to know how many times the "a" then "b" button were pushed by each worker in order to pay them for their effort. I'm not sure the best way to create this survey/game. Perhaps one can imbed a flash or java game into the qualtrics or mechanical turk set up?
