Create a quick landing page / optin page (EASY job)

This is a SUPER easy job.. I would do it myself but I am on vacation and left my main computer at home. I simply need someone to create an optin page like either of these for me; [login to view URL] or [login to view URL] I will give you my optin code to use (instead of what is already there) Its from aweber so when someone registers they join MY mailing list Again this is a SUPER easy project, I need it done TODAY.
