I need a PHP form with one checkbox and five text inputs. Each value should be recorded to a Wordpress database utilizing the get, update and add metadata Wordpress functions, which will allow the values to be unique to the currently logged in user. The forms should also display the current info in the database corresponding to each field. The update button should refresh the page so that the new information is displayed in the input fields, etc..
