Design in PSD and cut to html

I have a new website design in psd but i need some further work in design to create some order process pages for our product packages and also for our affiliate signup process. I have attached the psd of our new design with an empty content area where the order process will be designed and I have included a pdf file with some instructions and clarifications as to what we need in the questionnaire people need to fill out when they sign up. There are a few steps involved in the sign up because of the amount of information we need to glean from our customers to provide our service. Then I need someone that can cut these psd pages into html. The other part of this project is relatively straightforward we need another page like this one [login to view URL] but we need some changes in the questions asked in this form. This form is calling for expressions of interest to become a reseller but the next form is where they will actually sign up to be a reseller after they have been vetted as an appropriate reseller by us. This project is urgent so i need someone that can work on it immediately and have it finished within a couple of days.
