Adding LightBox style form to webpage.

On our webpage [login to view URL] we want to take off the contact page and replacing it by a inner pop-up, lightbox style contact form. The best example is : [login to view URL] --> Contact Us link. We need fields : Name, Email, Phone, Message, Send Button . On the bottom our contact info, which is found on our contact page. [login to view URL] And a confirmation message as well. That's it! I'll let you know the email where this needs to be sent by PM. It needs to be the same quality, with stylish fields and button.( I do not want to see this [login to view URL]) No childish programing/design please. This should take 1 hour. Needs to be done within the next 10 hours.
