Need someone to help me create a simple uploader form that will store data into my db and send me an alert email with form input info and download link for uploaded files and a page with pw to login to view data and let me download uploaded files. sample form: [login to view URL] sample data page: [login to view URL] (missing some data - customer name , company name, phone) my budget for this project is between 50 - 75 USD need this to be done asap so the bid with lowest price and fastest turnout time will get my project.
