Debug PhP to MySQL information entry form

hello, I have been working on a project for several months now that requires information to be input into a PHP-based webpage and then moving/entering the information into a MySQL database. During a recent set of updates/improvements by a past coder, a couple of items no longer inserts the appropriate numerical values from the entry form into the database. The input boxes are on the page, it's just the numbers are not reaching the appropriate database tables. When adding a new user: 1. the add new user script does not assign the user or a value to the "user_security_roles" table. In fact, the script should ask, via a checkbox list, for the desired security role. 2. When clicking on the "students" radio button, a box is available to sign the number of hours to be completed. The problem: this numerical number does not end up being inserted into the "student" table. to view this site in my sandbox area please visit: [url removed, login to view] username: rac1 password: rac1 It is also very possible that I have absolutely no idea what I'm talking about and might just be slightly more involved than I perceive it to be. You will see by my ratings that I'm fair and understanding of my own limited knowledge. If the problem legitimately ends up being much more then a fair and reasonable bid would suggest, then bonuses would certainly be in order. I am all about finding coders that I wish to create continual relationships with. I prefer to work fast and I like using the chat feature for immediate question/answer/results exchanges. 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
