Calendar Issue

I have a form that when the user inputs the "order date" or the "authorized date", the calendar pops up, but I can't see the arrow images to scroll month to month? I have similar calendar tools on my site that work and the arrow image for this page are uploaded to the server, but I'm not sure why this doesn't work for this page. I am looking for someone to fix this problem with a new calendar or explain what I may be missing? I have attached the html page and the css style sheet.
