Contact Form with Drop Down Email Selector

I have an existing contact form that I would like to add a feature to. I would like to add a user selectable drop down menu for about 5 items - with a style that is consistent to the design setup. IE. Support Warranty Sponsorship Billing etc. Each item will direct the contact form to a specific email address. So If they select Support it would go to EMAIL ADDRESS_1 for example.....or if they selected Warranty it could go to EMAIL ADDRESS_2, etc. etc.. The drop down menu needs maintain consistent visual appearance to the other form items in the contact form and be consistent across ALL major browsers. I have attached a screenshot of the form as it is now, the new drop down menu item will go below the PHONE value and above the MESSAGE value. The drop down should have an appearance similar to the one found here: [login to view URL] I have attached a PDF of the [login to view URL] file currently being used. If you need to see the HTML file or CSS files being referenced for the style/design, let me know. This should be a quick project and completed within a few days. If it takes longer than 5 days then I will not be able to leave a good review.
