Hi, I specialize in creating custom-made, interactive PDF forms. I had a look at the files you shared. Your forms seem to be the result of a scan. Do you have the actual documents used to create them? That would make editing them much much easier. If not I suggest to re-create your forms from scratch in Word, then generate PDF files from them, add the form fields and any scripts required to make them work as they should (to clear the form, submit it, etc.). I will then deliver to you the source documents as well as the resulting PDF files and explain how to edit them in the future. I looked at your attachments and this is the way I would go. I can re-create the forms in excel and create a template file for you. There are two things you can do with this. With a template file, every time you open it you will get a unfilled form which you can either save as a pdf or use electronically by filling in the boxes. It can then be edited easily from excel. The other way would be to upload the form from excel to a pdf. You can use Adobe Writer then to edit everything. Personally I would use excel initially and then upload into pdf to protect the logo.
