Data Entry - Move PDF text to Word Template

I need someone able to move data from a PDF into the appropriate spots in a word template. I have attached an example of the PDF and the text moved into the word template. You must have access to a PDF program that allows you to copy text, and at least microsoft word 2007. There are approximatey 30 of these PDFs to convert, all on average the same length as the example ones attached. You must have a good understanding of english to be able to think about where the text is going. I would expect completion within 7 to 10 days maximum.
