Convert Excel Form to HTML5 and JavaScript Offline Mobile Application with Offline Storage

Project Description: I have a flight log in Excel format that needs to be converted into an HTML5 Mobile Application with offline storage capability. The input form should be mobile-friendly, but the printable version should be the same format as the Excel flight log. The application should allow multiple logs to be created, saved and edited. Mathematical functions in the Excel flight log should be used as a basis for creating the app functions. All times are in Zulu (GMT). Deliverables will include the HTML5 Mobile Application files. The developer will be selected based on understanding of HTML5 mobile app development, familiarity with use of aircraft flight logs, ability to communicate effectively in English. If this project is successful, there will be a follow-on project to development an online application (written in PHP and MySQL) to allow synchronizing log information to a central location. The developer should be aware of this follow-on project, but should not include it in his/her proposal.
