Create a very simple app that walks the user thru a series of questions and checklists. Keeps a running total of the percentage of correct and in correct answers as the user runs thru the "session." This app will be used on a tablet only and by a therapist asking a series of questions. We have all the questions outlined. We have all the mockups finished as well in pad's. I have attached the jpeg version of the mockups only for review. As well as the checklist so you have an idea on the number of questions. Its quite a list. This is a very simple app and we have a very small budget as well. Please keep that in mind when bidding. We also have a very quick turn around time needed. We need this done in a matter of days not weeks.
