I have an existing form for lab / diagnostics requests. Im looking for someone to redesign the lay out from one full page to back to back half page, similar to a doctors prescription pad.
Here is the pad. I'd like to see a design that is back-to-back so that it can be shrunk down to roughly the size of a standard prescription pad. I'd also like the "We Do Housecalls!" to be featured prominently at the top of the page, together with our phone numbers.
Other contact information such as our address, website, email address, etc. can go somewhere else in the design if need be, as long as the housecall info is at the top.
It would be ideal if the sections of packages/panels, blood chemistry, hematology, and microscopy could be on the front side of the pad. They are the most commonly requested tests.
Lastly, the address will change as we are moving soon. I will give the new one once I know exactly what it is.
