Create a design documet from screenshots and raw text data

I need a professional designer to design a document based on screen shots and raw text data. Think of this as a brochure. I will then print this document in high quality and give it as a present. ##### IMPORTANT ####### - TURN AROUND TIME FOR THIS IS 3-4 DAYS. I NEED IT BEFORE SUNDAY. DO NOT BID ON THIS JOB IF YOU CANNOT WORK TO THIS TIME FRAME, OTHERWISE I WILL BE AN UNHAPPY CLIENT. Please take a look at the attached document. As you can see, I have screenshots of a website. This is only a sample - I have another document with all of the screen shots for the winning freelancer. I have the underlying raw text data as well. You do not need to create or design anything new, you just to design from the screenshots I give you. I need you to create and design me a document which includes these designs as a document I can then print as a high quality document. I would prefer this to be a MS Word document, because I can then edit it and change it around if I need to. The budget for this is between $30-$100 and I expect there to be 8-13 pages for you to design.
