Add a form to existing website

Greetings of the day, I already have a wordpress website in which restaurants/hotels can book STAFF for their business. For example a restaurant is looking for a new cook or a waiter [login to view URL] I want to update the website with a new function, so people who are looking for a job (example a cook) can send me their informations. Like a job interview I need to create a form in which a user can do or select the following things: (It should work like a feedback from which will pop inside the website form the right or left site) - Upload a Image - Select category - Cook, Waiter etc. - Give his name, addres s, e-mail, age, phone number - contac t - Upload an attachment - like job experience or a vita - Can select on which date he is available to work - calender - Can select in which area he likes to work - Germany, Austria etc. All this information should be stored in a database so i can easely search for the right person. Inside this database it should be possible to search for: - cooks/waiters (category) who are looking forward to work in Germany (category) and are free in this especially date (category) after that all results should be shown. It should be than possible to select some of the search results (check box) and send a mass e-mail to all of them. It would be great if we could show a small preview image of the users inside the database or where ever you think its possible. Please show me links of work you have done already.
