Create a form in a form designing tool (training provided)

This project is to take paper forms and design electronic versions in the Medforce FormsCenter application. The help manual for this application is attached. We will provide training and a system on which to build the form. If you are familiar with form building concepts, like selecting data types, field widths and aligning fields then you should be able to learn to use FormCenter fairly easily.

The four forms you will be creating are attached in PDF form. Examples of fields on the forms:

Patient Name - Text field 200 characters

Room # - Whole number field

Weight/Height - Text fields 15 characters

Co-Pay amount (27 fields) - Decimal number

And so on... There are also static text labels and lines/grids to be drawn on the print form.

For each form you will be making an entry form and print form. The entry form is where you drop the data entry fields. The print form is where you lay out the form so it prints nicely and looks exactly like the original paper form.

Requirements:

1) Familiarity with form designing concepts such as selecting data types, field widths and aligning fields.

2) Great communication skills - this is key!
