Update a Microsoft Word document to create a form that allows for variables in the document to be populated. Customer name, address, etc.
Desire is to create a form with the variables that will auto populate the word document these are just some of the fields in the final document. This needs to be a well designed form, that can be updated. Once the form is created it should also auto-update each of the fields and print all pages but this forms page. The document needs to be protected from changes, on ALL items other than the custom fields. You should not be able to change fields on the document, only in this form that will then populate the document.

The document is password protected, MS word 2007, saved with 2003 compatibility. Please PM for password. This should not take very long, the document is created, I just want the ASK variables to setup and populated.
