Create mailing list in Excel

What I need done: There is a publicly available web page with about 100 names and addresses. I need that data entered into an Excel spreadsheet in a usable format. The data is here: [login to view URL] Fields required in the spreadsheet: Each column should have a header. The headers should be: First Name, Last Name, Title, Hospital or Company Name, Address 1, Address 2, City, State, Zip, Work Phone, Home Phone, Cell Phone, Email Address Time to complete once bid is accepted: 2 business days
