Contest Brief

I need a lead generating Landing page for my site www.ArtificialGrassAZ.com which is a WordPress sites. I'm sending paid google Adwords traffic to my site now and I'm not getting enough leads.

I really like this landing page http://lifecellskin.com/ExclusiveOffer/ Please use as an example of what I need, but designed for Artificial Grass. I need the UI to be Responsive Design for Desktop, Mobile and Tablets. It needs a lead form field in which you can use same code used on Homepage now. I'll need design and coding to my site.

    www.artificialgrassaz.com. Denny just gave me a screenshot of an example I liked and what I need, but using my images, logo I provided.
