Project Brief:

We are a small family owned/operated garage door repair & Installation company. We are trying to have a fillable .PDF form created that would allow our technicians to record all the measurements required to order a new garage door for our customers on their tablet while in the field.

We have included a broad idea of what we are after. We are hoping that someone with a lot more creativity could not only spruce up the design/layout, but also make the form a fillable one. We are not looking for someone to just add fillable forms to our concept. We are not fond of our design (or lack there of) and would like more done to it. The concept .PDF is there to just give an idea of the fields that we require.

For all the below notes to make sense, please see the attached .PDF “Door Order Form.”

On the supplied .PDF concept, all boxes in blue would be a required field that the technician needs to input a value before completing the form. The items in red are for reference only and should not be on the form. We would like for items A through H to populate on graphic in its corresponding location when the technician inputs the value in the below required fields. We have attached the graphic for your convenience. Again, if you can create a better graphic, please feel free to do so.

The items located in the grey box will require the technician to input both the recommended door opening width and heights. The drop-down menus will need to have the following selectable options;

No Opener – Operated Manually
Overhead Opener
Side Mounted Opener
Other

“Recommended Track”
12”R Track
15”R Track
Low-Headroom, Front Mounted Spring(s)
Low-Headroom, Rear Mounted Spring(s)

In either “other” option above, an additional box would have to open allowing the technician to add additional notes/comments.

Regarding the left Jamb, right jamb, and header fields, the technician will be required to input a value. Then, from a drop down menu, select whether wood fill will be required or not. If there is, an additional option would appear, requiring another value (in inches).

Lastly, the two questions near the bottom of the form, if answered yes, would open up a comments sections that would allow the technician to clarify.

The second page is for reference only. This will be page two (or the back of the fillable form) and will be used as a guide for the technician. No active fields are required.
